What the Day Program Offers

Green Care Farms offers a meaningful experience built around belonging, purpose, and contribution. The program is designed to support people living with dementia through outdoor activity, social connection, routine, and time in a real farm setting that supports dementia care and care-partner respite.

A Typical Day

A typical day begins with arrival, coffee or tea, and time to settle in together. Daily tasks are discussed and shared based on the season, the group’s goals, and each member’s needs and interests.

Activities may include planting, garden care, harvesting, observation, and light hands-on work. At midday, members enjoy lunch together, often in connection with produce grown or harvested on the farm.

The afternoon may include games, crafts, invited guests, hydration breaks, and low-intensity movement or relaxation. The day closes with a group reflection on what went well and what comes next.

Who the Program is For

Cognitive impairment or neurocognitive disorder

At this stage, Green Care Farms is best suited for people who are living with a cognitive impairment or neurocognitive disorder.

Outdoor activity and physical mobility

Participants should be physically mobile and can walk and sit or stand with a maximum of one-person assist, and enjoy outdoor activity, nature, or meaningful hands-on work.

Daily participation needs       

Participants should be able to take medication independently, with cueing if needed, and can eat and drink independently. The program is intended to be inclusive of people diagnosed at younger ages, including those living with young onset dementia.

Targeted Outcomes

  • Social connection and social skill-building

  • More time outdoors and maintaining physical activity

  • A sense of belonging, kinesthetic memory, and nutrition and hydration

  • Retention of fine motor skills, self-worth, purpose, confidence, care-partner respite, and continued living at home where possible

Staff and safety

Green Care Farms staff are chosen carefully, screened thoroughly, and trained in dementia care and program policies before working with members. The program takes a thoughtful, quality-of-life-centered approach to outdoor participation.

Staff stay close with the group, support members in tasks they are comfortable with, and spend time helping new members settle into the space and routine.

What families should know

Transportation is arranged by members and care partners. The program takes place in an outdoor farm environment, some terrain may be uneven, and there is always some risk in outdoor activity, but participation is supported thoughtfully and carefully.
